1. Select the primary parish, school or organization that you will volunteering or working at within the Diocese from the drop-down list.

2. Select your role from the drop-down list. Please note that the role of "Educator" should ONLY be selected if you are a Licensed School Educator or Administrator working in a Catholic school within the Diocese.

3. Select a participation category. If you do not know which checkbox to select, please contact your local safe environment coordinator or the Diocese.
